As a volunteer for the Volunteer Center of Sacramento, I consent to perform assigned services strictly in the capacity of a volunteer and not as an employee of said agency, or of any other person, firm or corporation. Participant, youth participant, and parent/guardian do hereby waive any and all claim for accident, injury or compensation of any nature arising from said services against the Volunteer Center of Sacramento or any affiliated organization, officers, agents, and employees.
